I am not familiar with the phrase "needle-faced". However, I assume it is
similar in meaning to "sharp-featured".
It would mean angular or pointed, but not necessarily "small".

Whoever wrote that got her name wrong. Her first name is spelled
"Courteney" (shake head and mutter "Americans!"), and she has added
her husband's surname, ergo "Courteney Cox Arquette."
