Just saw an episode of Entourage, where Turtle and Drama are playing a
videogame and the following vonversation occurs:
- So you're a little glitch bitch!
- Blame the game makers, don't blame the player, hater!
Is that second phrase common, or has it just been made up when they wrote
the script for the show?

It's relatively common (in one form or another), though I've never heard
it in reference to an actual game.

It looks like a variation of the expression, "Don't hate the player,
hate the game." There's a brief article on the expression and
"player-hating" at Wikipedia at <http://en.wikipedia.org/wiki/Player_Hater>.
