Not necessarily in non-rhotic British English. ;-)  The accent is, of
course, different.

It depends what you think the 'A' stands for. Me, myself, personally, I
would substitute 'H' (for 'head') anyway, as it has presented me with an
irresistible mental image ever since I heard the riddle "What goes 'Ha,
ha, thud'?" as a child.
